Overview:

The EMS and Trauma Provider Grant Program assists private and public organization in improving and expanding the emergency medical and trauma system in Colorado. Applicants must have as their purpose the provision of emergency medical and trauma services in Colorado to be eligible. This grant program does not fund land acquisition, capital construction, fire apparatus, uniforms, disposables and infrastructure.

Cash Match:

The grant program requires a 50% local cash match, unless approved for financial waiver.

Mailing your grant application:

You can electronically submit your agency profile and grant application with no need to mail them. However, we do need signed originals of the W-9, Vendor Disclosure Statement and the Attestation (if you applied for a financial waiver, we need that original signature as well). You must mail these to arrive by 5 p.m. on Tuesday, Feb. 15, 2011 to:
